Doping – approved Medical Declaration form 2011
In preparation for the new membership year, British Swimming have approved the Medical Declaration form for 2011.
Please note the following:
Medical Declaration forms are only to be completed for those swimmers that enter ‘NATIONAL’ EVENTS.
Please do not send in medical declaration forms for swimmers that compete at club galas and club open meets. Incorrect forms will be returned to the club.
